do hereby certify that my child is in good health and physical condition and that I know of no physical defects, illnesses or medical conditions that would interfere or prevent my child from participating in the activity I have registered my child for below and have not been advised otherwise by a medical practitioner.
On behalf of my child and myself, I understand and assume the risks to have direct or indirect contact with individuals which have been exposed to and/or diagnosed with one or more communicable diseases, including but not limited to COVID-1 9 or other medical conditions, diseases, or maladies, and/or mutation or variation thereof does exist and it is impossible to eliminate such risk that I or my child could become infected through contact with or close proximity to an individual with a communicable disease. Further, I and my child agree to follow all the rules and regulations promulgated in accordance with the applicable New York State Department of Health Guidance to be enforced by the subject organization. The Mattituck Park District and its employees shall not be liable for the contraction of COVID-l 9, or any other communicable disease, arising from the participation in this program.
On behalf of my child and myself, I do voluntarily accept and solely assume all risks and dangers involved or incidental to the sports activity for which I have registered my child, whether occurring on the Mattituck Park District or Mattituck Park District/Mattituck School District activity/playing area, or on any part of the Mattituck Park District/Mattituck School District owned activity/playing area. I agree that my child is voluntarily participating in the activity knowing and appreciating the risks and danger associated with this activity.
On behalf of my child and myself, I understand and assume the risks inherent in the sports activity in which my child is participating including but not limited to the risks of injury from accidents, actions of other participants, topography of the activity/playing area, weather conditions, defects or injury resulting from equipment, balls, bats, nets, playing surface and defects and obstacles where the activity is being played.
I voluntarily agree to indemnify, hold harmless and release the Mattituck Park District, its trustees, commissioners, officers, employees and agents and the Mattituck-Cutchogue Union Free School District, its trustees, officers, employees agents and representatives from and against any and all causes of action, liabilities claims, suits, costs and expenses, negligent acts, and injuries and damages of any kind whatsoever resulting from or in any manner related to, in whole or in part, any permitted activity or my child’s participation in any permitted activity.
I have carefully read and fully understand all of the above terms and conditions, and freely and voluntarily agree to be bound to those terms and conditions.
