I understand that I am responsible for having my dancer dropped off and picked up on time. Early drop off between 8:45-8:59am and/or late pick-up between 4:01-4:15pm is available at no extra charge. We cannot accommodate earlier drop-off or later pick-up at this time; please make arrangements for your dancer.
I understand that that there are no refunds or credits given if my dancer is unable to attend.
I understand that an email will be sent mid-June from the studio, as a reminder regarding my registration (including what to bring and wear, times, dates, etc.). I understand my dancer does not need specific dance shoes to participate in the summer dance camp. Bare feet or clean indoor running shoes are acceptable.
Terms and Agreements

Please read carefully. By submitting this form, you agree to the following;
I, the undersigned and/or parent/guardian, do hereby give permission for my daughter/son/ward/self to attend and participate in the programs offered by Atyeo’s Dance Studio. I hereby acknowledge that by attending, teaching and/or participating in the program(s), there is a possibility of physical illness or injury to my daughter/son/ward/self; and I do hereby for myself and all others who might have a similar claim waive, release, and forever discharge any and all rights and claims for damages, which may arise now and in the future, against any and all damages which my daughter/son/ward/self may sustain and suffer while attending and participating in the program(s) at Atyeo’s Dance Studio. Emergency Personal Protective and First Aid Equipment will be available for staff and participants on site, if and when needed. I, the undersigned and/or parent/guardian, do understand the importance and responsibility of hand hygiene and will guarantee that my daughter/son/ward/self can demonstrate proper hand hygiene.
I, the undersigned and/or parent/guardian, and my daughter/son/ward/self, will respect the property and its belongings. I, the undersigned and/or parent/guardian and my daughter/son/ward/self, will not park or drive on the grass. I, the undersigned and/or parent/guardian, will ensure that my daughter/son/ward/self is properly dressed and prepared for class, including hair. Outdoor street shoes will not be worn in the hallway, change rooms, washrooms or studios. I, the undersigned and/or parent/guardian will ensure my daughter/son/ward/self, will practice safe and appropriate cell phone and camera use in and around the studio, and at all studio events. Cell phone use is prohibited in the change room and washroom areas.
I, the undersigned and/or parent/guardian, understand that all programs begin and end at their scheduled time. Proper athletic wear is needed for Summer Dance Programs. Please do not wear dresses or skirts. There are no rescheduled classes or refunds/credits for classes you, personally, do not attend.
I, the undersigned and/or parent/guardian, understand that Atyeo's Dance Studio reserves the right to cancel the enrollment of a child for the following reasons: Non-payment or excessive late payments; Disrespecting and/or the unwillingness to follow our Terms and Agreements; A child has special needs which we cannot adequately meet with our current staffing patterns; Physical and/or verbal abuse to staff or children, by a parent/guardian or child.
I, the undersigned and/or parent/guardian, allow Atyeo’s Dance Studio to photograph and/or videotape my daughter/son/ward/self for advertising purposes in local newspapers, on their social media, on posters, in television appearances and/or in dance class. I, the undersigned and/or parent/guardian, understand the full name of my daughter/son/ward/self will not be published without my permission.
You will be contacted by phone or text message if any Summer Dance Programs you registered for have been cancelled due to weather.
In the event that our studio is forced to close due to an outbreak of disease, illness or otherwise; all dance families will be contacted by email and regularly updated on our plan to move forward. No refunds will be offered, however, credit will be honored and updated to your dance account with regard to this situation only.
We will make every effort to work with the parent/guardian of children having difficulties in our programs. We are here to serve and protect all children; and children displaying disruptive behavior which has been determined to be upsetting to the physical, emotional and/or well-being of any other child, or teacher, will result in consultation and possible suspension.
This is a facility that cares for the well-being of all our dancers, staff and parents/guardians, and the environment that we provide. It is unacceptable to enter our facility under the influence of drugs and/or alcohol. Our expectations are that you and your family members are not under the influence of drugs and/or alcohol when you arrive on the property. Please arrange to have another adult/guardian drop-off/pick-up your dancer, if necessary. Otherwise, we accept your withdrawal from our dance studio. Also, discussions regarding how to buy, sell and/or the usage of drugs and/or alcohol are not permitted.
The use of profanity and speaking negatively about another child/dancer, parent and/or teacher (whether at our studio or another studio) is not permitted. If there is an issue, please speak with the director/owner, Miss Tracy.
No parent/guardian/teacher and/or dancer may use the name (Miss Tracy and/or Tracy Atyeo), the studio name (Atyeo's Dance Studio), and/or the studio logo(s), without the director's written consent. All parents/guardians/teachers and/or dancers cannot impersonate Miss Tracy, Tracy Atyeo and/or Atyeo's Dance Studio, to get access to online dancewear sites, open a social media and/or online account, register for dance competitions/workshops, etc. Please understand this is a chargeable offense.
I, the undersigned and/or parent/guardian ensure that anyone responsible for dropping off and/or picking up my daughter/son/ward/self, at the studio and/or any studio events, will be made aware of all the studio's Terms and Agreements, Schedules and Timings, Rules, Expectations, etc. I, the undersigned and/or parent/guardian, take full responsibility for anyone who is responsible for dropping off and/or picking up my daughter/son/ward/self.
