To complete this form, you need to have the following documents: 

1. A unique Student Number can find in the LOA or Invoice
2. Flight information (upload require) Student Name, Arrival and Departure flight information will be reviewed.
3. A picture of the student (Size:640pixels X 640pixels is maximum) for Student Attendance
