  • The position I am applying for may require the operation of a company vehicle or my own personal vehicle on behalf of Central Contractors. Having and maintaining a satisfactory driving record is a condition of employment.

    I hereby authorize Central Contractors to obtain my Motor Vehicle Record (MVR) from any state or federal agency, or from any third-party vendor authorized to provide such information, both prior to hire and periodically thereafter during my employment.

    I further agree to immediately report to my supervisor any license suspension, serious accident or offense, or any other condition that may affect my ability to drive on behalf of Central Contractors.
