  • I understand there are risks associated with having artificial eyelashes applied to and/or removed from my natural eyelashes. I understand that the eyelash extensions will be applied to the natural lash as determined by the technician so as not to create excessive weight on the natural eyelash thereby preserving the health, growth, and natural look of the client’s natural eyelashes. I understand as part of the procedure eye irritation, eye pain, eye itching, discomfort, and in rare cases eye infection may occur. I understand and agree that if I experience any of these issues with my lashes that I will contact my technician and have the eyelashes removed immediately and consult a physician at my own expense. 

     

    I understand that even though the technician may apply and remove the eyelashes properly, that adhesive materials may become dislodged during or after the procedure, which may irritate my eyes or require further follow-up care. I understand and agree to follow the aftercare instructions provided by my technician. Failure to follow the aftercare instructions can cause the eyelash extensions to fall out. I understand that in order to have the eyelash extensions applied to my eyelashes I will need to keep my eyes closed for a duration of 60-180 minutes during the procedure.

     

    I also understand that I will need to be lying in a reclined position. Any medical conditions that might be aggravated by lying still for a prolonged period may mean I will not be able to have the procedure performed on my eyes. This agreement will remain in effect for the procedure and all future procedures conducted by my technician for one year from the date of this signed form. I understand that this agreement is binding and that I have read and fully understand all information listed above. I represent that I am over the age of 18 years. If below 18 years of age a parent or guardian must also sign this form. 

     

    I understand that Lash Mahal has a 3-day policy. This policy means that I have 3 days to receive a free touch-up if my eyelash set is not to my liking. I acknowledge that any day after the 3rd day a $45.00 touch-up fee is applied. I also acknowledge that there are no refunds. I acknowledge I have reviewed all pricing. 

     

    I also understand that if I am late, I am waiving my right for my free touch-up within 3 days. We do not honor free touch-ups if you are a first-time client & coming in with lashes from another establishment or are a returning client with lashes from another establishment. We do not honor free touch-ups if after the 24-hour period you have not washed your lashes. We do not honor free touch-ups if you have put on any mascara or any type of strip lash and/or cluster. If you cancel or reschedule your free touch-up within 6 hours or no show you are no longer eligible for a free touch-up. If I am late for my appointment my free touch-up is not honored. For 30-minute appointments I am aware there are no free touch-ups. I am aware a $20 non-refundable, non-transferable deposit is required to book, and must be sent at the time of booking. Deposit must be sent within 24 hours of booking appointment or it will be canceled. The remaining balance will be paid at the time of your appointment. The only form of payment accepted is CASH or zelle. There are NO REFUNDS. NO EXCEPTIONS.

  • PAYMENT

    A $20 non-refundable, non-transferable deposit is required to book, and must be sent at the time of booking. Deposit must be sent within 24 hours of booking appointment or it will be canceled. Deposit must be sent via zelle. Zelle/quickpay: 630-423-6539. The remaining balance will be paid at the time of your appointment. The only form of payment accepted is CASH or zelle. There are NO REFUNDS. NO EXCEPTIONS.

  • ARRIVAL

    Please aim to arrive 5 minutes before your scheduled appointment time with clean/makeup-free eyes and lashes. There is a 15-minute grace period. Please notify us if you are running a few minutes late. After 15-minutes your appointment will be canceled and you must reschedule. If you arrive after your scheduled appointment time, it may not be possible to extend the time available for your booked service; if your service is shortened due to your late arrival, you will still be charged the full cost of the service.

  • CANCELLATION + NO-SHOW

    As a courtesy, appointment reminders are sent out 72 and 24 hours either by text, email, or both. If an appointment is canceled or rescheduled less than 24 hours before your appointment, you will be charged 50% of your service as a cancellation fee. If you reschedule or no show your appointment within 4 hours of your scheduled appointment time you will be charged 100% of your service as a cancellation fee and a $10 fee will be charged at the time of rebooking. If you reschedule for a 3rd time, a $25 non-refundable deposit will be required for all future appointments. A non-refundable credit card deposit of $20 for all new clients will be required at the time of booking. This is to protect your time that I set aside especially for you, as well as to protect our time which books up quickly. In turn, we promise to keep appointments on time, give you our full and undivided attention, and of course, the best lashes Naperville has to offer!

  • SICKNESS

    If you, or another person in your household, has an infectious or contagious illness, please contact us as soon as possible to reschedule your appointment for a later date. For your safety and that of staff and other clients, please do not come to your appointment sick. If it is assumed you are currently sick, your appointment may be cut short or canceled and rescheduled for when you are healthy again. A one-time allowance of a last-minute cancellation or reschedule will be permitted for sickness. After that, the cancellation and no-show policy are in effect.

  • REFUND POLICY

    You are paying for artist time, product and other expenses used to provide you with a service. No refunds will be given for any reason on services or products. If you are unhappy with a service, you may contact us within 72 hours (3 days) of your appointment to discuss your concerns and if a fix can be done to address your concerns, it will be done so with a complimentary 30 minute express touch up if it is at the fault of application or product. Any concerns addressed after 72 hours of your last appointment, or if you failed to follow proper aftercare instructions, will be charged at full price for the service.

  • CHANGING YOUR APPOINTMENT

    A minimum of 24 hours notice is required to reschedule or cancel a booked appointment without penalty.

  • REFILLS

    You must have at least 50% of your lashes left in order to be considered a refill appointment, otherwise it will be considered a FULL SET. Anything after 3 weeks is considered a new FULL SET.

  • OTHER ARTISTS' WORK

    We do NOT fill other artists work. If you currently have eyelash extensions on from another lash artist, your current extensions will need to be removed. Due to the unregulated nature of eyelash extension training, certifications, and state board regulations, there are a lot of lash artists that have been improperly trained. At Lash Mahal, our priority is your natural lash health and eye safety. Thus, if you are coming in for a touch-up with another lash artist work, then you agree that your current extensions need to be removed and a new full set will be applied at the appropriate full set price. This will be discussed and fully explained to you prior to any service being performed.
