MUFFY SKATES: Roller Skating Lessons: Liability Waiver
The Participant and/ or their parent(s) or guardian understands, acknowledges, and agrees that roller skating is an inherently dangerous and physically demanding activity. Participating skater assumes full responsibility for any risk of injury, property damage, or death while participating in activities at this establishment.
Participant and/ or their parent(s) or guardian hereby waive any andall claims including, but not limited to: bodily injury including death and property damage they may have against (MUFFY SKATES, its parents, subsidiaries, partners, affiliates their subsidiaries, officers, directors, employees, agents, and volunteers from arising out of Participant’s use of the MUFFY SKATES roller skating lessons.
Participants and/ or their parent(s) or their guardians hereby release and hold harmless (MUFFY SKATES, its parents, subsidiaries, partners, affiliates their subsidiaries, officers, directors, employees, agents, and volunteers) from arising out of Participant’s use of the MUFFY SKATES roller skating lesson program from any and all liability for any loss, damage, or expense that assigns and legal representatives may suffer as a result of Participant’s use of the MUFFY SKATES roller skating lesson program, from any cause whatsoever, including but not limited to, negligence or breach of contract on the part of (MUFFY SKATES roller skating lesson program, its parents, subsidiaries, partners, and affiliates, and all sponsors of MUFFY SKATES, in the operation, supervision, design, maintenance, or condition of the roller skating surface, and all surrounding areas.
With full knowledge of the risks involved, I hereby release, waive, discharge MUFFY SKATES/LatinExpressions Dance/ Skateland, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to `unintentional exposure or harm due to COVID-19.
BY SIGNING THIS RELEASE OF LIABILITY AND WAIVER OF ALL CLAIMS, I UNDERSTAND THAT I AM RELEASING MUFFY SKATES AND ALL SPONSORS OF THIS ROLLER SKATING LESSON PROGRAM FROM ANY CLAIMS FOR NEGLIGENCE AND DAMAGES CAUSED MUFFY SKATES AND ALL SPONSORS NEGLIGENCE AND AGREEING TO THE OTHER TERMS IN THIS DOCUMENT.
