WOF Application Form

Warrior of Faith is a ministry. In our day to day ministry work we come in contact with vulnerable adults and children. Unfortunately the world we live in can be cruel and dangerous, therefore we require a background check for every potential new member.

We believe in love and grace, but we still have a responsibility to keep the vulnerable adults and children we come in contact with safe. A criminal record does not in itself mean an application will be denied, however, offenses against children or vulnerable adults are offenses we feel should be brought to the attention of the applicants local chapter president.

In connection with my application to become a member of Warrior of Faith, I understand that a criminal background check will be required and that report may contain information about me relating to my criminal background history or other background checks. Such reports may be obtained at any time after receipt of this disclosure and authorization and if I am accepted as a member and throughout the course of my membership, as permitted by law and unless revolked by me in writing.

Warrior of Faith LLC also reserves the right to share my report with the president of the chapter I am applying for membership under.

I understand that I have the right, under written request, made within a reasonable amount of time after the receipt of this notice, to request the disclosure of the nature and scope of any investigative consumer report to Protect My Ministry Inc., 14499 N. Dale Mabry Hwy., Suite 201 South, Tampa Florida 33618.

It should be noted that because of the nature of the Warrior of Faith Motorcycle Ministry, some professions may be considered to be a conflict of interest and membership would not be offered in such cases.

I understand what I have read, and I am ready to start my journey of becoming a member of the Warrior of Faith Motorcycle Ministry.

I do not make this decision lightly, and I understand as a member I am giving of myself, my time, my treasure, and my talent to serve the Lord Jesus Christ.

I understand I am joining Warrior of Faith MM and Warrior of Faith MM is not joining me.

I understand the need to leave all doctrinal differences at the door and to focus on Christ Crucified as I ride and minster the Gospel of Christ.

I understand that my support and commitment is to my local chapter, and to the members I will be true, settling all disagreements and conflicts within the ministry and in accordance with Matthew18.

I have read everything on these pages. If I had questions, I have asked those questions and they have been answered to my satisfaction. Once Warrior of Faith receives your application form, your Chapter President will be sent an invoice in the amount of $25. +tax for the background check. Once paid, we will submit your information. It normally takes a few days to come back.*
