Warrior of Faith is a ministry. In our day-to-day ministry work we may come in contact with vulnerable adults and children. Unfortunately, the world we live in can be cruel and dangerous. Therefore, we require a background check of every potential new member. We believe in love and grace, but we still have a responsibility to keep the children and vulnerable adults we come in contact with safe. A criminal record does not in itself mean an application will be denied, however, offenses against children or vulnerable adults are offenses that we feel should be brought to the attention of the applicant’s local Chapter President.

In connection with my application to become a member of Warrior of Faith, I understand that a criminal background check will be required and that the report may contain information about me relating to my criminal history, or other background checks. Such reports may be obtained at any time after receipt of this Disclosure and Authorization and if I am accepted as a member, throughout the course of my membership, as permitted by law and unless revoked by me in writing. Warrior of Faith also reserves the right to share my report with the President of the Chapter I am applying for membership under. I understand that I have the right, upon written request made within a reasonable amount of time after the receipt of this notice, to request disclosure of the nature and scope of any investigative consumer report to Protect My Ministry, Inc., 14499 N. I understand that if I request a copy of the findings that one will be mailed to me at the provided address. I agree to allow Warrior of Faith to do a criminal background check and understand all findings will be kept strictly confidential and only shared with the President of the Warrior of Faith chapter that the applicant resides in.

Once Warrior of Faith receives your application form, you will be sent an invoice in the amount of $25. +tax for the background check. Once paid, we will submit your information. It normally takes a few days to come back.*
