If you wish to appeal your citation(s), please complete the form below and include all pertinent information you deem necessary to appeal your case and mail it back to us within 15 days from when our letter was sent, or you may bring it to our office. You will be notified via mail of your hearing date. If you would like to appeal your citation(s) but you are unable to attend an appeal, you MUST submit a notarized written statement with any pertinent information you deem necessary to appeal your case via mail or in-person within the 15-day allowable period from when our letter was sent.
