I/We understand and agree that mentoring services may relates to and may involve any/all of the following, including: ’phone/online video call +/- email mentoring/advice; in-person mentoring, support and coaching/guidance; observing/shadowing, and/or coaching on pet animal training; observing/shadowing, and/or coaching on pet animal behaviour consultation(s) and behaviour modification lesson(s); reviewing +/- editing of documentation, for example client or vet follow-up, case notes, or case reports; other (as specified). I/We acknowledge that the nature of this may change or evolve, and that this applies to current/multiple/future mentoring interactions:

I/We understand that mentoring fees are payable at the time of each/all booking(s), and are non-refundable. I/We agree to send my payment by BACs directly: Santander bank account: name - Inspiring Pet Teaching; sort code - 090128; a/c no. - 58024255; ref - your name. PLEASE ARRANGE YOUR MENTORING SLOT(s) PRIOR TO SUBMITTING YOUR FORM/PAYMENT. (Please provide the service to me immediately, i.e. within the next 14 days. I/We acknowledge that I will lose my right to cancel the contract, or receive a refund, if the Service commences within 14 days of this Booking.)
