to participate in swim lessons. I understand that I should seek the advice of a physician before commencing any exercise routine. I understand that swimming is a hazardous activity with inherent risks. I assume on behalf of the above-named child/children all risk of loss, damage or injury arising out of participation in swim instruction with Swim with Patrice, EPP, Inc. or Patrice Pennington. I assume any and all risks of injury related to swim instruction or pool facilities. This waiver applies regardless of whether individuals are injured at time of lesson or as a result of the lesson. I agree not to sue or otherwise attempt hold Swim with Patrice, EPP, Inc. and Patrice Pennington liable for injuries regardless of cause. On behalf of myself, the above-named child/children, my heirs, legal representatives and assigns, I hereby knowingly and intentionally hold harmless and release Swim with Patrice, EPP, Inc. and Patrice Pennington, together with any sponsoring groups or clubs, and their respective directors, officers, agents, affiliates, employees, contractors, officials, sponsors, and volunteers, from and against any and all claims, actions, suits and demands pertaining to or arising directly or indirectly out of any damage, loss, illness, injury, or death as a result of participation in these lessons. I authorize any representative of Swim with Patrice, EPP, Inc. and Patrice Pennington to have participants treated in any medical emergency during participation in swim lessons and agree to pay all costs associated there within. This waiver is intended to be be as broad as is allowed under the applicable law and applies to any and all claims of damage, regardless of whether they were caused by the negligence of any employee.
Lessons typically run 4 days in a row.
Cancellations must be made 24 hours in advance to avoid paying for your reserved class time. We can expect to swim in the rain, but not in the thunder. There is one cancellation per registrant.
You will recieve a payment link the week before class begins. Cash, check, zelle or venmo payment should be sent before the first class. Required waiver for participation.
