$50 Late Fee will be assessed for forms not submitted by April 22, 2025. A $20 fee will be assesses per player substituted or withdrawn after Tuesday, April 22.

Players MUST be chosen from your eligibility list and MUST BE LISTED IN ORDER OF ABILITY.

This year all girls' and boys' tennis teams may request to have up to two (2) additional singles players added to the regional qualifying site draw. These additions may happen only if a region's draw is not MAXED OUT and FACILITIES and TIME allow. Therefore, it is extremely important that alternates are correctly listed in order and by ability. Check the MPA Tennis Bulletin to further understand this new machination allowing additional singles players at the regional qualifying events and the extra fee required ($20 per player) to make such additions.

We request the following additional tennis athlete(s) be entered in the singles qualifying tournament if the size and logistics of the draw so permits. These players will also be the first to be considered as substitution(s) if replacement prior to the qualifying draw becomes necessary.

Coaches' Eligiblity

Students participating in the Singles Tournament must be accompanied at the tournament site by a coach who meets the MPA Coaches' Eligibility Standards.
Entry/Withdrawal Information

A school is not required to sends individuals the State Singles Tournament. However, if your school returns this form it is expected that your coaches or a school delegate will attend ALL seeding meetings. It is also assumed that your coach/delegate and designated players will attend all rounds of the state singles competition that players' success dictates. If for some reason these commitments cannot be kept, please contact the appropriate MPA Tennis Committee member and consult the Tennis Bulletin to determine the consequences for your school or individual player. Registered singles players may be withdrawn or replaced by designated alternates for a fee of $20 per withdrawal/replacement prior to the draw at the Regional Seeding Meetings. AFTER THE REGIONAL SEEDING MEETING, NO SUBSTITUTIONS WILL BE ALLOWED. Once a singles entrant has been assigned, a line on the qualifying or championship drawsheet his/her non-appearance will result in a $75/$125/$150 progressive fine levied against his/her school.
