Communication Plan
Desired communication for telehealth services is videoconferencing technology or a phone call. If there are problems with a videoconferencing session because of technical difficulties, the session will be completed by a phone call.
If having a videoconferencing session, it is advised that clients connect 5-10 minutes prior to the appointment time and check whether the internet speed, webcam, and microphone work properly.

Either Doxy or Zoom will be used for the platform for videoconferencing sessions. By signing this form, you agree and acknowledge that sessions will not be recorded by either party unless there is written agreement to do so.
