Have you had previous microblading, tattoo, or any other permanent makeup on your brows (or lips if booking lip blush)?
Please note that Madeline only works over previous if it is at least 50% faded, and if the shape/color is something she feels comfortable working with. All new clients are considered initial sessions, not touch ups.

Please read and agree to the following terms. These are required to receive a permanent makeup service. Your acknowledgement signifies your agreement and understanding of these terms. If you arrive to your appointment and it is indicated that you are in violation of the requirements, your appointment will be cancelled and a 50% fee will be charged.

I am not pregnant or breastfeeding.
If I have diabetes, cancer, heart condition or am currently undergoing treatment for cancer, I will provide my doctor's approval. I understand that my results may be affected by these conditions.
I have not used Accutane in the past year.
I will discontinue use of skincare products containing retinoids, acids, AHA/BHA, and acne treatments on the forehead area for 1 month prior to my appointment. If booking lips, I will discontinue use of these products two weeks prior.
I will avoid chemical peels, microdermabrasion, or other facial treatments for 1 month prior to my appointment if booking brows, or for 2 months of booking lips.
I will not have botox / fillers for 1 month prior to my appointment if booking brows, or 8 weeks if booking lips.
I will avoid brow waxing, tinting & threading for two weeks prior to my appointment (does not apply for lip blush).
I will avoid sun and tanning two weeks prior to my appointment.
I will not consume alcohol, caffeine, blood thinners (including pain relievers), or vitamins/supplements 24 hours prior to my appointment. If booking lips, I will not consume alcohol in the 5 days prior to my appointment.
I will follow the 5 day precare regiment if booking lip blush, and understand that if I arrive to my appointment with dry, chapped, cracked or irritated lips, my appointment will be rescheduled and I will be charged a cancellation fee.
I will follow the aftercare instructions provided to me and understand that failing to do so WILL impact my healed results.
