In consideration of the services rendered by GOLDENLEASH LLC to my pet(s), I acknowledge reading, understanding, and accepting the statements herein. Please acknowledge the following policy statements by checking the boxes provided. If you do not agree to all statements, please do not complete this form, and contact Golden Leash LLC directly for more information.
Liability Waiver
I understand certain “activities” that my pet may participate in, including walks, one-on-one training, within the the client's personal residence, on site, in public places and transportation, if necessary, to and from Golden Leash LLC involve risk and possible injury, including but not limited to: Exposure to parasites, viruses, and other medical conditions passed from pet-to-pet or person-to-pet; sprains, strains, bites, broken bones; Motor vehicle accident during transportation; and fatigue, dehydration, nicks, cuts, or death. I further understand that not each and every potential risk can be listed above but, nonetheless agree that the benefits associated with pet socialization and training outweigh the possible risks, therefore, I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify Golden Leash LLC and its agents, successors, heirs, from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my pet’s participation in activities at Golden Leash, including those allegedly attributable to the negligent acts or omissions of Golden Leash LLC their staff.*

I am aware of and agree to release the care of my pet under the following terms and conditions.
Allergies, Special Diets, and Medications:
I agree that I will disclose to Golden Leash LLC and their associates any and all allergies, medications, or special dietary constraints my pet may have/require if necessary during their service dates. I understand that a failure to disclose these special needs may result in illness, discomfort, strange behavior or death.*

I am aware of and agree to release the care of my pet under the following terms and conditions.
Photographs & Statements
I authorize use of my pet's visual image(s) and my personal statements (testimony) in newsletters, posters, official websites, and other materials. ( See our instagram @palo_alto_paws )*

I am aware of and agree to release the care of my pet under the following terms and conditions.I request that photos of my pet not be shared with the public in any shape or form.
Dog Aggression
I affirm that my pet has NOT bitten any person or dog in the past 3 years. If my dog HAS bitten another person or dog within the past 3 years, I affirm that this person or dog will not be present during the sitting, or on the property of service.*

I am aware of and agree to release the care of my pet under the following terms and conditions.
Payment Agreement
Golden Leash LLC accepts check, cash, and nearly any form of electronic payments. I agree to pay the service rates in effect for my pet’s participation in activities at or with Golden Leash LLC. I further agree to pay for any additional services requested such as drop-off, pick-up service, pet sitting and/or any additional tasks. I acknowledge that payments for in home dog sitting and dog training is due in full one(1) day before first day of service per house-sitting, first day of service per pet training, and there are no acceptions.*

I am aware of and agree to release the care of my pet under the following terms and conditions.
Rights to Refuse & Discontinue Services
I understand that Golden Leash LLC and its associates reserve the exclusive rights to decline participation or to terminate services within reason in activities at/in the care of Golden Leash LLC to any pet at any time during a service.*

I accept and agree to these terms.
Attorney Fees, Applicable Law & Venue
Should I, Golden Leash LLC, or anyone acting on their behalf, be required for any reason to incur attorney fees and costs to enforce or defend this agreement,I agree the losing party shall indemnify and reimburse the other for such fees & costs. Further, I agree and understand that any disputes arising out of this Agreement will be decided pursuant to the laws of the State of California and the venue shall be in Santa Clara, San Mateo, or San Francisco County.*

Cancellation Policy
I acknowledge that Golden Leash LLC's cancellation policy is as follows: Cancellations made during any stay will not be refunded. Refunds are granted if written notice is given 48 hours before the service starts. If canceled later than 12:00 pm two days before the service begins, a 50% refund is available. If the cancellation is requested 5 calendar days from the date the service was requested, a 100% refund is available.*

I accept and agree to these terms.
Valid Dates
These agreements, waivers, and authorizations will remain valid and in force as long as and whenever my pet participates in any activity at or with Golden Leash LLC and its participants.*
