VOLUNTEER INFORMED CONSENT AND RELEASE

I wish to volunteer at WARM.

I understand that the nature of volunteer activities which are typically performed by volunteers and which may be performed by me as a volunteer, may involve physical activities, contact with unindentified and unfamiliar persons, foreign and unidentified substances, travel to and from various unspecified locations, and other potential risks of injury.

Knowing this, I wish to volunteer and hereby assume the risk of any accident or injury to person or damage to property which I may sustain in the connection with my participation as a volunteer for WARM. In addition, I hereby release and discharge WARM and its directors, officers, partners, affiliates, agents, representatives, successors, and assigns from any and all liability or responsibility for such accident, injury, or damage.

I agree that I will only perform volunteer activities that I am comfortable doing and I have no known condition that would impair my capacity to perform the services for which I have volunteered. I affirm that I have never been convicted of a crime, child abuse or neglect, child pornography, kidnapping, rape or sexual offense. I grant full permission for WARM to use photographs of me and quotations from me in accounts and promotions of volunteer projects and for training purposes.

I understand that WARM performs sex offender registry checks on all volunteers, criminal history checks on volunteers working with children or one on one with residents, and on all volunteers providing transportation services. My registration is considered incomplete until the review and approval of my applicable checks and notification from WARM of the process' completion.
