Information to include in your application

Please ensure you have the following documents readily available before you begin this application form, as you may need to refer to them. You will be asked to upload electronic (i.e. scanned) copies of these documents in the last section of this form. If you do not have access to electronic copies, you can provide printed copies to the school before your first day of enrollment. There is also a $100 non-refundable application fee that can be submitted with the application, at the front desk, or over the phone. Once you have submitted your application add reception@fosbreacademy.com to your email contacts so your acceptance letter doesn't end up in your spam folder!
