List of documents and photos that are required.
Below is a list of documents or images that may be reuqired for you to turn in with your application. Highlited are those who are required for each item. You can upload image files below. Or email them to me at downtown@morristownchamber.com Thank you.
Driver’s License – Provide a copy of your current driver’s license. (everyone )
Certificate of Insurance – A basic liability policy with a $500,000 liability with the City of Morristown and the Morristown Area Chamber listed as additional insured. Farmers and those selling edibles such as eggs, breads, cakes, etc. must have food liability coverage.( everyone )
Certificate from TN Department of Health – Required of those selling prepared/ready to eat foods such as sandwiches. ( Food Trucks, hot served foods, specialty foods )
Plant License – If you are selling plants, provide the appropriate license that has been approved by the TN Department of Agriculture. ( farmer, organizatioin )
Commercial Kitchen Certificate – Document showing that your product was produced & packaged in a commercial kitchen that is inspected by the TN Dept. of Agriculture, the TN Dept. of Health, and/or federal agency. (food trucks, hot served food )
Retail Meat Sales Permit – All meat sold must be processed through a USDA inspected facility, and have a USDA seal on the package. A TN Dept. of Agriculture Retail Meat Sales Permit must be posted where meat sales occur. (farmer)
City of Morristown Food Truck Permit – An approved permit indicating that your food truck has met all city requirements. ( food trucks, hot searved foods )
Photographs of goods Artisans wish to sell – Photos of the goods artisans wish to sell are required of that type vendor applicant. At least three photos of representative work must be provided. ( goods, arts, crafts )
