School Based Traineeship or School Leaver Expression of Interest

This form is to gather information about a potential school-based traineeship or for any student considering leaving school prior to the completion of Year 11 or Year 12.

This form can also be used by employers who are seeking or employing a school-based trainee.

This form may be completed by a student, parent/carer, careers advisor, KTS representative and or an employer.

Please complete as many applicable fields as possible.

The following questions should be completed by the students wishing to undertake the School Based Traineeship. If the student is not available, please complete as best you can.
