- Want to book a session with your bestie? Fill out separate forms with your shared availability and mention each other's names.
- Submitting this form does not guarantee an appointment.
- Do not send a deposit until I offer you a date and time!
My hourly rate is $225 and my minimum charge is $200. Be prepared to send a $100 deposit to secure your appointment time. Deposits are non-refundable and go toward the cost of your tattoo.

A thorough description of your idea*
Lay it on me! Give me all the details. Example: I want a T-Rex wearing roller skates sitting on a mushroom in full color, approximately 4x4” on my outer right thigh. I have some other tattoos in that area and I will attach a photo below. Do you have more than 1 idea? Include size/placement/details for each idea here; no need to fill out a separate form.

SHOP POLICIES
- We encourage mask-wearing, but we are no longer requiring masks to be worn at the shop. I will wear a mask at all appointments.
- If you feel sick or have been close to someone who does, please let me know as soon as possible and we can reschedule. If you show up with symptoms you will be charged to reschedule.
- Please arrive alone; no +1's at this time. Let me know below if you'd like to make arrangements to have someone accompany you.
- Do not send a deposit until we have confirmed a date!
