    • LIGHTING ROUND: For a limited amount of time (May 12-May17 12:59pm/NOON), we are accepting an additional round of applications. You will need to submit this form completely by that time. Applications are time stamped to ensure accountability on both ends.
    • This application allows you the opportunity to apply to one or more of these programs, using one form.
    • Read the application carefully prior to submission and make sure the information you provide is complete and accurate.
    • In addition to questions, there is also a place in the application to upload an electronic copy of your most recent high school transcript, as well as your Standardized Test Scores (if you choose to do so).
    • All applicants must have someone submit a letter of recommendation on their behalf at  https://bit.ly/taphealth_letter  (Additional  instructions given below).
    • Once you submit, you will receive a confirmation e-mail.

A couple questions to get us started:

    • Personal Information
    •  -
    • Demographic Information
    • The next 3 questions in this section relate to demographics. The information helps us collect data for our evaluation records, and will not affect your selection into the program. The questions should be answered as to how you best identify yourself out of the choices provided.

    • Parent or Legal Guardian
    •  -
    • Education

    • Though it's not the most important part of your application, we are interested in learning about your standardized test scores. If you've taken one or more, you have the option to share your ACT and/or SAT score for additional consideration.

    • Attachments
    • School/Educator Contact
    • In order to apply for this program, you should only do so with a school Educator (Guidance Counselor, Teacher or other administrator) aware of your application. This individual will be contacted by us if we have any questions or verification needs. We will also let them know the final status of your application. This person will be considered your School Contact.

    •  -
    • Extracurricular Activities
    • We would love to learn a little more about who you are! In the boxes below, please list a few activities you are or have been involved in. Please share years (or dates) of participation, as well as what the activity entailed. You do not have to complete every box, but we ask that you share at least two.

    • Essays
    • We know essays can sometimes be tough, but please don't let that stop you from applying! We want our Selection Committee to get to know you in a more in-depth way. You don't have to be a great writer to submit an essay - just write with sincerity. The two essays will be reviewed based on multiple factors, including content, passion, grammar, readability, flow and just how well you generally answered the question. One question will be the same for everyone (Personal Statement), and another question will give you a choice in topics (Writing Supplement).

There is nothing additional to upload - we have provided enough space within the application. We suggest having your essays ready (checked for grammar, word count, and for the content you want to say) in a program like Microsoft Word or an e-mail before you paste into the essay boxes.

    • Personal Statement: Please share your a personal statement about yourself. You can include why you are considering being a healthcare professional one day, and/or why you feel you would be a good fit for this program. You can also opt to share a personal statement that simply speaks to who you are. Please make this statement no less than 250 words and no more than 650 words.

Be awesome. Be creative. Be yourself.

    • Writing Supplement: Choose one out of the five (5) essay prompts below. This should also be no less than 250 words and no more than 650 words.

Be awesome. Be creative. Be yourself.

    • Letter of Recommendation
    • Please ask an adult you trust to write you a letter of recommendation. You should give this person as much time as possible to submit their letter by the application deadline. It is best to choose someone familiar with you as either a student, work, sport, club, volunteer activity, or other extracurricular you have been involved in. You can also ask someone else who may have unique insight to why you'd be a great fit for the profession of healthcare one day.

A few reminders before you submit:

      • Letters of Recommendation will be submitted separately from your application and should come directly from the recommender.
      • When making your request, they should submit on your behalf at:  bit.ly/taphealth_letter
      • If for any reason your Recommender changes after you've submitted your application (but before deadline), please let Heleena McKinney know the new name to expect on your behalf at: hmckinney@healthcollab.org
