  • I, , grant permission for my child, , to participate in this LOPC event at the church campus. This activity will take place under the guidance and direction of volunteers of the LOPC Sheds of HOPE ministry.

  • As a parent/guardian, I remain legally responsible for any personal actions taken by the above named minor participant.


    As a parent or guardian of the participant, I give my permission for my child or ward to register for and attend this event and further, in consideration of the acceptance of the Office of LOPC of such registration, I agree, individually, and on behalf of my child/ward, to the terms of this release of liability.

     
    I, the lawful parent/guardian of the participant named above release from all liability, and indemnify and hold harmless the Pastor of Lake Oconee Presbyterian Church ("the Pastor"), both individually and in his representative capacity, Lake Oconee Presbyterian Church, ("the Church"), the Session, Employees of the Church, Volunteers of the Church and anyone else acting on behalf of the Church from any and all liability, actions, causes of actions, claims, demands, judgments, damages, costs or expenses including attorney fees, known or unknown as this time, arising out of or in any way related to any injury or illness, death, bodily injury, personal injury, pain and suffering incurred to my child/ward while participating in or traveling to or from the activity.

     
    I, furthermore, release from all liability, and indemnify and hold harmless the Pastor of Lake Oconee Presbyterian Church, both individually and in his representative capacity, Lake Oconee Presbyterian Church, the Session, Employees of the Church, Volunteers of the Church and anyone else acting on behalf of the Church from any and all liability, actions, causes of actions, claims, demands, judgments, damages, costs or expenses including attorney fees, known or unknown as this time, arising out of or in any way related to any injury or illness, death, bodily injury, personal injury, pain and suffering incurred to my child/ward while participating in or traveling to or from the activity.

     
    I appoint members of the Session and agents who are acting as leaders of the activity as my attorney-in-fact to act for me in my name and on behalf, in any way that I could act if I were personally present, with respect to the following matters if any injury, illness and medical emergency occurs during the activity to give any and all consents and authorizations to any physician, dentist, hospital or other persons or institutions pertaining to any emergency medications, medical or dental treatments, diagnostic or surgical procedures or any other emergency actions as my attorney shall deem necessary or appropriate for the best interest of my child/ward.

     
    The powers and authority granted to me, herein, may be revoked by me, by written notice delivered to the Pastor or his agents who are acting or have previously acted hereunder. Without any such written notice, this power of attorney shall not be affected by my disability, incapacity, or adjudicated incompetence. This power of attorney shall lapse automatically upon completion of the activity and return of my child to the ending place.

     
    Parent/Guardian expressly agrees that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Georgia, and that any portion of the agreement is held invalid, it is agreed that the remaining valid portions shall remain in full force and effect. Parent/Guardian further states that he/she has carefully read the above release and knows the contents of the release and signs the release as his/her own free act.

     
    This release contains the entire agreement between the parties to this agreement and the terms of this release are contractual and not merely recital.

    I hereby grant LOPC permission to copyright and use photographs/videos taken at Sheds of HOPE events of the minor designated above in any manner or form for any purpose lawful at any time. I waive any right that I may have to inspect or approve the finished product or written copy, that may be used in conjunction therewith, or the use to which it may be applied.

    1. Participants must stay for the entire event unless accompanied by a parent or guardian.
    2. No foul language, drugs, alcohol, tobacco, or weapons are permitted.
    3. Participants must respect the rights and properties of others.
    4. Participants and parents/guardians are responsible for any and all damages caused by the participant.
    5. Failure to abide by code may result in participant removal, by parent, from the event.
