Date - fill in the date of the activity, within the Sunday-Saturday range you selected above.

Employer Name - share the name of the employer for that activity.

Employer Phone - type in the phone number of the employer. If the activity was online or was an online application enter "N/A".

Employer Address or Website URL - type in the address of the employer or, if it was an online activity, the website URL (ex. https://www.westmiworks.org).

Activity Type - indicate the type of activity, i.e. interview, resume, follow up, internet, etc. If other, please list.

Actual Time Spent - list the amount of time spent for the activity as hours in 30-minute increments (30 minutes would be .5 hours) Ex. one and a half hours equals 1.5.
