Feel free to complete this form as soon as possible to get your K9 approved prior to needing assistance. Per our bylaws, we are unable to provide reimbursement for vet bills that have already been paid and must pay the veterinarian's office directly.

Documentation stating the specific time period that the K9 served is required to be approved for assistance. This must be uploaded under "Proof of K9's Service" below. This documentation must include the specific dates of service. For example, August 2018 - June 2023.

Please note, for a K9 to be considered for retirement assistance, they must have been owned by a casino or resort during their years of service. K9s that were personally owned during service and K9s from contractors are not eligible at this time.

If you do not provide sufficient documentation, we will provide you with our written eligibilty policy which outlines exactly what is required.

We will make every effort to provide assistance to approved K9s. However, all assistance is dependent upon current budgets and obligations.
