General Office Policies:
-If you are late for an appointment, your treatment will still end on time so that the next appointment may begin as scheduled. If you are more than 15 minutes late your appointment will be automatically canceled and be considered a "no show". In both of these instances your service will be charged in full.
-We reserve the right to refuse or discontinue service to anyone demonstrating behavior that is perceived to be inappropriate or disruptive to our healing atmosphere.
-All product returns must be accompanied by a receipt. Product returns will only be accepted within 15 days of original purchase. All returns will be refunded with a gift card.
-Prices and services are subject to change at any time.
-Gift cards are sold in $5 increments, except at the online store where they are sold for various set amounts. Gift cards are not refundable or redeemable for cash and may not be used for gratuities. Promotional or discounted gift cards cannot be used towards Spa packages or other specials.
-To ensure the safety of children and the enjoyment of all clients, we ask that parents or guardians make other arrangements for their children while they are receiving services. Children must be 14 or older to receive services and must be accompanied by an adult at all times until the age of 16.
Privacy Policy:
I understand that as part of my care, this organization originates and maintains health records describing my health history, symptoms, examination and test results, diagnoses, treatment and any plans for future care of treatment. These records are secured and will not be shared without my consent.
I understand that this information serves as:
-A basis for planning my care and treatment.
-A means of communication among the many healthcare professionals who contribute to my care.
-A source of information for applying my diagnosis and surgical information to my bill.
-A means by which a third-party payer can verify that services billed were actually provided.
-A tool for routine healthcare operations such as assessing care quality and reviewing the competence of healthcare professionals.
I understand that I have the right:
-To request restrictions as to how my health information may be used or disclosed to carry out treatment, payment or healthcare operations – and that the organization is not required to agree to the restrictions requested.
-To revoke this consent in writing, except to the extent that the organization has already taken action in reliance thereupon.
Cancellation Policy:
-Appointments cancelled 48-24 hours prior to your scheduled start time may incur a fee equivalent to 30% of your service. As everything is fully customized for you, we begin preparing for your session the day before.
-Appointments cancelled within 24 hours of your appointment start time will be charged 50% of your scheduled services fees.
-Clients must communicate a need to reschedule at least 24 hours prior to their appointment to avoid a cancellation fee. I know things happen and I do my best to accommodate.
-Repeat offenders or those whose card on file fails to cover fees will be required to purchase a gift card prior to scheduling an appointment to ensure cancellation fees are covered.
Consent to Treatment:
-I acknowledge that my skin might experience temporary irritation, tightness, redness or slight swelling which usually dissipates within 72 hours depending on skin sensitivity.
-I acknowledge that if I am allergic to one or more ingredients in the products used, I may experience allergic reactions. I have notified Dr. Melissa of all known allergies.
-I acknowledge that if I fail to use sunscreen, I am more susceptible to sunburn, skin damage & hyperpigmentation. I should avoid excessive sun exposure especially between 10am-2pm.
-I acknowledge that this treatment is strictly elective cosmetic procedure and no medical claims have been expressed or implied.
-I acknowledge that I should avoid the use of Retin-A type products, aggressive exfoliation, waxing, and products containing acids that are not part of the recommended regimen for 2-4 weeks following treatment.
-I agree to use any post treatment products that I am given as directed
-I consent (to the best of my knowledge) that the answers I have given are correct and that I have not withheld any information that may be relevant to my treatment. I give consent for this and all future treatments
-I release Dr. Melissa Durfey DACM, LE and her staff of any liability associated with any injuries and /or current and future conditions resulting from the skincare procedures or products.
