2021-2022 Season Discounts:

Sibling Discount: $150 off 2nd and 3rd player and $500 off 4th player.

Goalie Discount: (Squirts and up) $200 off

Discount(s) will be applied to 5th and 6th Payments as necessary.

The player listed above commits to play with the Delaware Stars for the 2021-2022 Season. I (we) understand that there is a $250 NON-REFUNDABLE fee (Mites $150) due at the time of signing this commitment letter. As the parent(s), I (we) agree to pay the Delaware Stars according to the payment plan noted above. The Delaware Stars Ice Hockey Association will be strictly enforcing the schedule above. If your payments are not made timely per the schedule above, your player will not be allowed on the ice for practice or game participation until your balance becomes current. I (we) have read and agree to the payment agreement as set forth in this document. I (we) understand that if PAYMENT IN FULL is not made on or before 11/1/2021, player(s) will be reported to the AAHA for non-payment and the Delaware Stars Youth Ice Hockey Association will pursue payment in a court of law.

Delaware Stars Youth Ice Hockey Association Letter of Commitment
I ____(player name)____, hereby accept a position with the Delaware Stars Youth Ice Hockey Association for the 2021-2022 Atlantic Amateur Hockey Association (AAHA) season.

We, the undersigned player and parents/legal guardians of said player, by executing this Letter of Commitment, agree to the following terms and conditions of this commitment.
1. We agree to remain with the Delaware Stars Youth Ice Hockey Association (DSYIHA) for the entirety of the 2021-2022 season.
2. We understand and agree that there are NO REFUNDS and that the fees for the entire season are due and payable in accordance with the DSYIHA's payment schedule upon signing this letter of commitment.
3. Except as set forth below, we understand and agree that if the above named player refuses to participate with the DSYIHA after signing this agreement that he/she is ineligible to participate with any other AAHA member program and must sit out remainder of the 2021-2022 season.
4. We understand and agree that a player who sits out due to refusal to participate, remains obligated to satisfy any outstanding financial obligations for the 2021-2022 season.
5. We understand and agree that a player who sits out all or a portion of the 2021-2022 season for any reason is not entitled to any partial or full refund or forgiveness of program fess or expenses.
6. Attendance for all practices and games is strongly encouraged. Team bonding and development occur at practices, and game success is reliant on every player. Extenuating circumstances and pre-planned absences are understood.
7. We understand and agree that no player from any USA Hockey Affiliate or District may register or participate on a team within the Atlantic District, if such player owes any financial obligation, including un-returned/borrowed uniform or equipment, to any previous team or organization registered within any USA Hockey Affiliate or District.
We, the undersigned player and parents/legal guardians further understand and agree that this Letter of Commitment may only be terminated by mutual consent between the Delaware Stars Youth Ice Hockey Association Board of Directors and the requesting family, and must be in writing. Upon such mutual consent, and payment of any outstanding financial obligations, the program will issue a proper AAHA Release for the player to transfer. Such mutually agreed transfers must occur prior to Decemeber 31, 2021.

PAYMENTS
Effective for the 2021-2022 Season.

Payments will be made via ACH. Invoices will be issued monthly to be paid NET 30.

Checks will be accepted.

Cash will be accepted on a case by case basis.

I, _________(parents)________ agree to pay the $250 NON-REFUNABLE deposit fee. I agree to pay as indicated above on this commitment letter contigent upon the Delaware Stars fielding teams under the jurisdiction of the AAHA. In the event that a team at my child's age level is not able to be fielded this deposit will be returned. I understand that all payments must be made on time. I understand that failure to pay the installment fees within 5 days after the due date, that my child will not be permitted to continue to participate in any aspect of the Delaware Stars program until payment is received, and late fees will be assessed.

This agreement WILL BE STRICTLY ENFORCED. Any requests for extenuating circumstances or changes to the posted payment schedule must be made in writing to delawarestarsicehockey@yahoo.com. Special payment plans must be approved and signed by the Stars President and Treasurer or they will be null and void.
