Parental responsibilities*

At least one parent is a Baptised Catholic (I include a copy of their Baptism certificate in later section of this form)To raise my child as a Catholic and bring them to weekly Sunday Mass.To attend a pre-Baptism course (details below).I live in the boundaries of St. George's Cathedral (section marked 'C' in map below) OR I attend weekly Sunday Mass at the Cathedral OR I have a particular connection with the Cathedral
Godparents:

Godparents must be Baptised and Confirmed Roman Catholics, who attend weekly Mass.There must be at least one Godparent (maximum number of Godparents is four).You may choose someone from another Christian denomination to act as a Christian Witness instead of a Godparent but there must be at least one Catholic Godparent.
Donation

It is customary for parents to express their gratitude for the Baptism by making a donation to the Church. The amount is to be decided by each family and varies according to one's financial circumstances. The suggested donation is £50.
