Public Health Permit Application
  • Here is a complete list of documents needed to start with DTLA kitchens, and obtain your health permit.

    Please fill out and gather ALL documents before starting to upload (You will not have the ability to save your work.

    California department of Public health requires to be involved.

    We will need 2 additional documents, AND to send the form below with the check to the CA Department of Public health.

    The local public Health office will need proof that the check has been cashed by the California DPH before granting you the permit. So this should be mailed first, in order to expedite the permitting process. Please monitor your bank account and send us the proof as soon as it hits your bank account. 

     

    For special licensing like cannery, dog food, dairy product, we will require additional permitting from CDPH. If you are unsure about your need for any specialty license: Please reach out by text or email: (310) 461 8200 or applications@dtlakitchens.com

    Ice Cream Vendors: You will use our own Frozen Dairy license, and only need the regular health permit from the County. You will not need to contact the state at this stage. 

    For wholesalers. Please upload: 

    1. Standard operating procedures for your product(s) in the format of your choice.

    For 3. 4. 5. Use DTLA Kitchens 3212 E Olympic Blvd Los Angeles CA 90023. 

    For 20. please select Manufacturing D. $524, and the total amount should be $624.

  • Food handler certificate is needed within 30 days. I

    At least 1 manager certificate, and all employees MUST have a valid food handler's card during their shifts.

    If you do not have the certificate, please register for the class and send us the receipt using the link below.

  • To complete your application, you will have to pay the application fee of $180 to the public health department.

    We will receive an invoice within 5 business days of sending your application. Please pay it promptly and send us the receipt to expedite the process.
