ATC Health Record
Medications require an Administration of Medication Form that is signed by your doctor. This form can be printed from our website. We ask that you bring both the medication and the form on the first day of the session. Medication and Administration Forms already at ATC for the 2020-2021 school year may be left for Summer Review & Enrichment.
Please list the names of all persons that have permission to pick up your child from Kindergarten Summer Review & Enrichment (grandparents, neighbors, etc) in the text box below. The teacher will need to see a photo ID when they arrive to pick up your child. To indicate that your child may only be picked up by his/her parent please type "Parent Only" and list parent names.
Summer Review & Enrichment cost is $125 per session.
If you are able to register for a session, consider your child successfully enrolled in that date. When a session reaches capacity it will be removed from online registration. Please call the ATC Office if you would like to be placed on the waitlist.
Your registration is not complete until we have received a non refundable deposit of $75 (the deposit will be applied to the session cost). You may pay online below with a bank transfer, credit or debit card, OR pay in person with cash or a check.
NOTE: You will be charged a 3% convenience fee for card transactions and a 1.5% convenience fee for bank transfers by the online payment company for this service.
Your total due by the 1st day of each session is $125.
