We've Survived Before: An Inter-generational, Youth-led Mental Health Intervention for LGBTQ+ Youth

I have reviewed the information letter, I have asked any questions I have about the study, and I understand the answers.
I understand that when I take part in these six workshops, I will be a participant in a research study called "We’ve Survived Before: An Inter-generational, Youth-led Mental Health Intervention for LGBTQ+ Youth"and I would like to take part in this study.
I understand that the workshops are being recorded and typed.
I understand that I can leave the study at any time, refuse to participate, or refuse to take part in any activities without any consequences.
I understand that even if I quit the study, the information I shared in workshops can’t be removed from the study because it is part of a group conversations, but if I leave the study before July 1st 2021 my name or fake name will not be used in any publications.
I understand that the information I provide will be used in research publications and to develop a mental health program for other queer, trans, and non-binary youth.
I understand that other participants will know who I am and may share that information even though they have been asked not to.
I understand that I can choose a fake name (pseudonym) or use my real name in the publications from this study. If I use my real name, people who read the publications will know I took part in this study and they will know that I identify as queer, trans, or non-binary.
I understand that if I share information about abuse, self-harm, or suicidality, Nathan Thompson (certified counsellor) will contact me, and this information may be reported.
I understand that I will be reminded of my rights as a participant, and that I can ask questions at any time.
