I parent of above mentioned child request that my son/daughter may please be part of tournament. I hereby agree that my son/daughter shall abide by the rules and regulations. I also agree that organizers shall not be responsible for any incident or injury caused to my son/daughter during practice/tournament. My son/daughter shall co-operate maintaining discipline.
