hereby agree that any/all videos or pictures taken during Infant Swimming Resource (ISR) lessons must be granted prior approval by, your instructor, Lauren Haza a fully Certified Instructor, prior to being published on any websites, including, but not limited to: Facebook, Instagram, Twitter, YouTube, Blogs, Personal Websites, Company Websites, etc.

I agree to ask permission and only take pictures or video on days when my child is practicing a learned skill. This is strictly enforced. Taking pictures or video during unauthorized times, may result in lessons being ended early for that day. Your certified instructor will detail which lesson day is approved for filming. Videos cannot include prompts or procedures that are proprietary Infant Swimming Resource, LLC trade secrets. Skills appropriate to film include:

• Unassisted floating

• Roll-back to float

• Unassisted swim-float-swim

• Swimming from the instructor and/or to the edge of the pool, bar or steps

I hereby consent to the possession and use of images, depictions, and video recordings of

I, with my signature below, acknowledge that such photographs and video recordings shall be the property of ISR and the above-named instructor and that these images will not be sold or reproduced in any way other than mentioned here. ISR and my instructor have the right to possess photographs and video recordings, and any digital images taken of my child during lessons for use in promotion or educational materials as follows:

• printed publications or materials

• electronic publications or presentations

• website

• social media pages including but not limited to Facebook, Instagram, Twitter, etc.
I understand that my child’s name and identify will be protected by only using his/her first name and age in months/years i.e. “Jake can float at just 7 months old” I authorize the use of these images indefinitely without compensation of any sort. All prints, digital reproductions, video recordings and copyrights shall be the property of ISR and your instructor named above. *
