  • This coaching agreement is entered into between Sarah Hamilton, DBA Balanced Approach Coaching and the above-named client.

    Fees

    The fee for the coaching sessions and/or package will be charged before services are scheduled.

    ALL SALES ARE FINAL. Refunds will not be issued.

    Cancellation Policy

    Clients maintain the right to cancel services at any time. Cancellations must be done in person or by email. 

    If you need to cancel a session, please notify Sarah Hamilton of Balanced Approach Coaching at least 24 hours in advance of the session date to reschedule. If this is not done, the session will either be dropped, or the client will need pay for an extra session.

    Services

    The client will be provided with the coaching services included in the package/service of their choice. The purpose of the coaching sessions is to provide the client with information based on their defined goals. This service includes but is not limited to: goal setting, identifying action plans, follow-up procedures to maintain client motivation, educating the client as well as asking and answering questions.

    The methods employed by Sarah Hamilton/Balanced Approach Coaching do not guarantee results. They are guidelines that the client must follow through on to see any type of change.

    Although Coach Sarah of Balanced Approach Coaching will do the utmost to provide clients with correct and up to date information, Coach Sarah is not a...

    -licensed medical physician

    -licensed dietician

    -licensed therapist or counselor

    Coach Sarah will give advice and offer suggestions, but will not give prescriptions or diagnose medical conditions. It is not Coach Sarah's responsibility to prevent or cure the client of diseases or illnesses.

    ALL materials provided by Coach Sarah/Balanced Approach Coaching are to be used at the discretion of the client and under the provision of the client's healthcare professional. These materials include but are not limited to: menus, foods lists, informative articles or documents, and recipes. 

    Each client is to take personal responsibility for their own health. You are advised to speak with your healthcare provider before changing your diet and lifestyle.

    With group coaching (via Facebook, Instagram or in person meetings) I acknowledge that I am signing up for coaching with Sarah Hamilton. I understand that any guests, admins, coaches, speakers, or other members in her group are there for support and encouragement only.

    The client agrees, therefore, to hold Sarah Hamilton and Balanced Approach Coaching free of any liability and responsibility for adverse reactions as a result of advice given in the coaching sessions.

    Termination

    Termination of this agreement may occur at the discretion of the coach when an impasse is reached with the client. As long as the client adheres to the coach’s guidelines and the coach adheres to professional practices and standards, the relationship will continue. Termination at the discretion of the coach cannot be refunded.

    Examples of terminable offenses:

    ·         Lack of payment

    ·         Continual lack of adherence to coach’s guidelines without reasonable explanation

    ·         Indifference on the part of the client to prescribed session action items

    ·         Repeated cancelling of sessions and check-in calls

    ·         Repeated disrespect of coaches time by arriving late to sessions

    ·         Sharing documents and notes with any outside party (all documents and notes given to client are property of Balanced Approach Coaching and are meant for client use only)

    Confidentiality

    Any information provided by the client to Coach Sarah is completely voluntary and not required of the client. This information includes but is not limited to: health diagnoses, current medications being taken, familial and other relational problems.

     Anything that is said or revealed in the sessions between coach and client is privileged information and will not be disclosed to any outside party. The exception is if the information revealed includes disclosure of illegal, unethical or criminal activities.
