LATE POLICY

If you are running late please let me know ASAP. 10 minutes late will result in a late fee of $10. Beyond 15 minutes late will result in cancellation if I can't accommodate you and your deposit will be forfeited. In the event that your appointment gets cancelled as a result, you may schedule another appointment but another deposit booking fee will be required.

RESCHEDULING

In the event you need to reschedule your appointment, a notice must be made at least 48 hours prior to your scheduled appointment. A one time transfer of your deposit will be allowed if notified at least 48 hours prior to appointment. The rescheduled appointment must be within the same month. Any requests to reschedule more than once for the same appointment will require another non-refundable deposit to reschedule. Rescheduling within 48 hours of your appointment will result in being charged 50% of the service you booked for. Deposit is forfeited.

CANCELLATIONS

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the artist's day that could have been filled by another client. As such, in the event you need to cancel your appointment, a notice of 48 hours minimum is required. Same day cancellations/ cancellations made within 48 hours of your scheduled appointment will result in being charged 50% of the service you booked for. Deposit is forfeited. No-call, No-shows will result in a 100% service charge. Deposits are NON-REFUNDABLE, no exceptions. Failure to comply with policy will result in not being able to book with me again.

CARD ON FILE

A credit card/ debit card is required to be held on file when booking. Please note that the card on file will NOT be charged when booking an appointment. The card on file is subject to charge only if any fees are owed due to violation of the Aesthetic Beauty by Tammy policies.

NO REFUNDS AVAILABLE FOR ANY SERVICES, NO EXCEPTIONS. If you experience an allergic reaction, please contact me and I will do a removal free of charge. Thank you for understanding.

I hereby agree to have eyelash extensions applied to my natural lashes. By signing this agreement, I consent to the placement and/or removal of the eyelash extensions by the certified professional.

I understand and agree to the after-care instruction’s provided by the artist. And for any unexpected circumstance that have happened due to my own mistake or failure to follow these instructions are in my own risk and at my own expense.

I understand that it is my responsibility to keep my eyes closed during the entire procedure until my technician addresses me to open my eyes. I understand that in rare occasions there are risks associated with having artificial eyelashes applied to and/or removed from my natural eyelashes. I further understand that in rare circumstances, some risks of this procedure may be but are not limited to eye redness, swelling of eyelids, irritation and/or discomfort. I understand and agree that if I experience any issues, that I will contact my technician and have the eyelash extensions removed and consult a physician at my own expense.

I understand that because of the natural lash cycle and wear and tear, I will need to maintain my extensions with touch up appointments usually recommended about every 2 to 3 weeks to keep them full.

I understand that there are no refunds for any services, no exceptions.

I understand and agree to all components of this consent form, and hereby release Aesthetic beauty by Tammy from any claims, demands, and damages of any nature. I agree to pay the full amount of this service acknowledging the extent in which the service is taken place. I am of sound mind and fully capable of executing this waiver by myself. I request and agree to the service(s) provided by Aesthetic Beauty by Tammy.
