High School Mission Team

Your student has applied to participate in our High School Ministry Team. This is a two-phase training program intended to train the students spiritually, in work ethics and skills, and interpersonal relationships.

We desire your assistance in this program through your honest evaluation.

Section one of this form will have a bearing on the applicant’s participation in the program; section two is intended to assist us in evaluating the participant’s strengths and weaknesses for affirmation and correction.
