In consideration of my employment by the Company, I agree to abide by its rules and regulations.
I declare each of the answers given to be complete and true to the best of my knowledge and am aware that any misrepresentation or omission will be cause for immediate dismissal. I authorize all schools which I attended, any former employers and all other parties to give information relative to my academic, employment record and personal data. I hereby release them and the Company from all liability for any damages whatsoever arising therefrom.
I am aware that if employment is offered, as a condition of employment I must be authorized to work in the U.S. and demonstrate that authorization as required by the Immigration Reform and Control Act of 1986.
As a condition of employment, I understand that the Company may investigate my financial and credit history. Should the Company require it, I also agree to submit to a post-offer pre-employment substance abuse test. I understand that refusal or failure to submit to such testing, falsification of a test, or a positive finding on a test will remove me from consideration for employment and may result in my discharge if employed. If employed by the Company, I also consent to random and reasonable suspicion substance abuse testing at any time.
As a condition of employment, I understand that the Company will require a criminal and driving record background check for all applicants, and that I will be asked to complete a seperate background check consent form as part of the hiring process. I further understand that a background check can take days to weeks to return results, and if hired prior to my completion of the background check, my employment is conditional on a satisfactory background check. If I have any questions or concerns, I will discuss with the Company's hiring manager.
I understand that my employment is at-will and for no definite period of time and may be terminated by either party for any reason or for no reason. I understand that this application is not, and is not intended to be a contract of employment, and that any statements which may be contained in policies, practices, handbooks, or other Company material do not create any guarantee of employment. I further understand that the Company has the right to modify, amend or terminate policies, practices, benefit plans or other programs with or without advance notice at their discretion.
