    MIHOA Game Officials Request Form

    Complete the form and submit a request to the MIHOA game Schedulers.
  • All teams must be registered with Missouri Hockey Registrar before officials will be assigned to a game.

  • Complete this form and check the MIHOA scheduling web site to confirm the game was entered correctly. If the game was not entered correctly, use this form to submit a correction.

  • Request submitted are for scheduling officials only.  Request to add, change or cancel games must be sent to the appropriate league, tournament, club or opposing team.

  • Add a New Request for Game Officials

    If you wish to schedule officials for a new game, you must submit the game thru the MIHOA Scheduling System. If you have an user id and it is not working, you can request the game using this form. If you do not have a user id and password to the MIHOA Scheduling System you can request a user id and password using the MIHOA Scheduling System User ID Request Form on the MIHOA web site.     If your NEW game request is less than 5 days notice, please select the option below and complete the form to request officials for the games with LESS THAN 5 DAYS NOTICE.

  • Cancel Officials for a Game

    Please provide the information to cancel officials for a game. The cancellation is only for the officials, it is NOT a notice to the league, tournament or opposing team that you are cancelling the game.
