Request for employee testing

Dear Medtester,
Hereby I would like to make an appointment for testing our employees for Covid-19 infection.
The number of employees that would like to be tested is roughly  Number*   

Hereby, I declare First Name* Surname*   as position manager/owner of the companycompany name*   that our staff does not have the possibility to do their work properly at home. Also, it is not possible to keep 1.5 meters distance from each other at all times. For this reason we want to test our staff preventively.
