    1. Read the Medication Profile in MAR to know what the client is receiving. 2. Verify that the Medication Administration Record (MAR) has the same medications listed. 3. Know the purpose and any instructions for each medication. 4. Check the client status for contraindications to giving the medications, such as illness, nausea, vomiting, diarrhea, or swallowing problems.

  • Have all the clients taken all medications as ordered from day(s) before?
  • Any client condition changes (If so, have you notified the nurse? or scheduled an appointment based on client condition change if necessary?)

  • Are the clients refusing any medications (2-3x/week)?
  • Is any of the clients taking PRN frequently and are follow ups being completed?
  • Are there any new meds that have been prescribed to any of the clients this week?

  • Are there any medications that have been delivered and has it been communicated on whatsapp to the team?
