INTRODUCTION:

Thank you for taking the time to complete this reference form. We value your input as we assess the readiness of our applicants and discern how to best prepare and equip them for long-term service amongst the urban poor. This form takes an average of 20 minutes to complete.

Work with Servants is as demanding as it is rewarding. Servants workers live with the urban poor, usually in cross-cultural settings, and together with both colleagues and neighbors, they strive to improve the wellbeing of their communities. In order for us to best prepare our applicants for such intense contexts, it is crucial to first acquire an accurate picture of the candidate's strengths, weaknesses, and emotional/physical health. In light of this, please complete the following form to the best of your knowledge, giving candid answers, which will be kept strictly confidential. The form will be read only by: the relevant team coordinators (sending team and team being joined), and Orientation coordinator.

This form can be completed in multiple sittings. At any point, you can save your work, leave the site, and then return later to resume completion. To do so, simply click the "Save" button located at the bottom of each page. In fact, it's a good idea to periodically save your work just in case your internet cuts out.
