INTRODUCTION:

Thank you for taking the time to complete this reference form. We value your input as we assess the readiness of our applicants and discern how to best prepare and equip them for ministry amongst the urban poor. This form takes an average of 20 minutes to complete.

Work with Servants is as demanding as it is rewarding. Servants missionaries live with the urban poor, usually in cross-cultural settings, and together with teammates and neighbors, they strive to see their communities transformed by Jesus. In order for us to best prepare our applicants for such intense contexts, it is crucial for us to first acquire an accurate picture of the candidates strengths, weaknesses, emotional and spiritual health, etc. In light of this, please complete the following form to the best of your knowledge, giving prayerful and candid answers, which will be kept strictly confidential. The form will be read only by: the relevant Linking (Sending) team coordinator, Orientation coordinator, and immersion (field) team coordinator.

This form can be completed in multiple sittings. At any point, you can save your work, leave the site, and then return later to resume completion.
