• Chapter Orientation Program Application

  • Instructions: This completed application must be submitted to the International Office by a chapter requesting authorization to conduct an orientation program. A copy will be sent to the Regional Office. Approval or denial of this application shall be determined by the Regional Director and communicated in writing to the chapter via the International Office Disposition of a Chapter request to Conduct an Orientation Program. Chapters are reminded that all forms of hazing in conjunction with membership initiation activities, to include but not limited to, physical fitness training and mental hazing is strictly prohibited. False information contained on this application will result in disqualification.

  • Instructions: This completed form must be submitted by a chapter to the International Office and Regional Office to request authorization to conduct an orientation program. This statement serves notice that Groove Phi Groove, S.F.I. prohibits all forms of hazing in conjunction with membership initiation activities, including but not limited to, physical fitness training or exercising, and/or mental hazing, and that any individual and/or chapter that violates this organizational policy on hazing is subject to suspension or revocation of membership and/or chapter privileges. Members who are financially active with both the International Office and the chapter may sign this form, while only those members who sign this form are authorized to participate in the orientation program. All orientation programs must be approved by the Regional Director or the International Office prior to starting orientation programs.
