RELEASE FORM: As the parent/legal guardian of the contestant whose name is stated above on this application at the Washington State Finals, I do hereby agree to abide by the rules and regulations, directives and provisions for winners and contestants as set forth in the official Cinderella Staging Handbook, International Productions & Publications, Inc, and The Washington State Cinderella Pageant.
I agree to hold International Productions & Publications, Inc, the host venue,and/or the International & Washington State Cinderella Girl Scholarship Program & Pageant, its directors and staff members harmless for damages and/or liabilities due to theft, accident, or injury during or resulting from any child’s participation in the Washington State Finals. I also accept responsibility for the regulation of hours of personal activities of the participant. I understand and agree that should an act of God (hurricane, fire, tornado, or any other natural disaster)cause the cancellation of the Washington State Finals, that said event will be cancelled without benefit of any refund—however, the States Finals will be rescheduled at another time/location. I also confirm that all the enclosed information and forms are true and correct to the best of my knowledge and belief, including birthdate, and city/state of residence.
I hereby have read and understand the rules and regulations governing the payment of scholarship awards, and all other prizes and awards. Also, if my daughter is one of the young ladies who receive any crown (Overall, Beauty, Talent/Tot Personality, Photogenic, or Cover Girl) or participates in the Washington State Finals, I give my permission for her picture and name to be used on social media, website, Facebook page and/or in futureadvertising for Cinderella purposes. I further understand that if my daughter is crowned Tot-Teen Overall, that she is required to attend the International Pageant. If she does not fulfill her obligation to attend the International Pageant, I understand her awards and title will be forfeited and given to the next candidate in line. In addition, I will not be entitled to any refund of States fees or expenses; and will be responsible for reimbursement to the state office for awards received. This also applies to state queens who resign prior to crowning their successor.
