SUBSCRIPTION AGREEMENT
I hereby agree to be a member of Cooperative of Allegro Microsystems Phils., Employees (CAMPE). I have read and fully understand the CAMPE PMS Video Online prescribed of prospective members and I understand the purpose and objectives of the cooperative. In connection with my membership, I hereby agree to the following terms and conditions:

1. To comply with the provisions of the Articles of Cooperation, By-laws and Policies set by the Board, the General Assembly and as well as the acts of duly constituted authorities, failure on my part to do so, the Cooperative, as its options, may:

a. Fine

b. Suspend

c. Expel me from membership, where all my shareholdings shall be answerable for my liabilities to the Cooperative of Allegro Microsystems Phils., Employees (CAMPE).

2. To participate in the planned thrift and savings program by:

a. Subscribing for at least 25 share value at Two Thousand Five Hundred Pesos (P2,500.00) and paying for the either in lump sum or in regular installments. If on installments, to pay at least the value of one share on or before the organizational meeting and the balance upon approval of the Board, of my application for membership in regular weekly, bi-monthly installment as selected in the DEDUCTION PER PAYDAY field.

b. Contributing to its share capital an amount equivalent to One Hundred Pesos (P100.00) for Monthly-paid employees and Fifty Pesos (P50.00) for Daily-paid employees for one year unless extended as declared by the Board.

c. Contributing to at least 2% monthly or at least 10% annually for every loan granted. Failure on my part regarding the financial obligation, unless excused by the Board, shall make me liable for a fine as stated on our operating policies.

3. To pay the membership fee of P100.00

4. To comply with the directives of duly constituted authorities as well as the decisions of the Board regarding the operating policies of the cooperative.

5. To continue with subscription of at least P100.00 for Monthly-paid employees and P50.00 for Daily-paid employees.

The provisions of this agreement, the Articles of Cooperation and the By-laws (explained to me, understand them and agree to abide all of them), in all of the above undertaking I am aware that the Board of Directors and it’s officers may impose sanctions against me or perform acts necessary to make the sanction/s effective without going to court. By submitting this form, I hereby I agree to all the terms and conditions of this subscription agreement.
