Passport Book for an Adult

Passport Processing Requirements

• Bring completely filled Application for a U.S. Passport Form DS-11
• Valid State ID or Driver’s License (copies not accepted)
• Original certified long form Birth Certificate or Certificate of Naturalization (copies not accepted)
• Must provide the applicant’s Social Security No.
• Must provide an approved passport photo (photo service available at the city)

  Passport Fees per Applicant

• Check or money order payable to: U.S. Department of State
  • $130.00 per adult passport book

• Check, money order, debit, or credit card (must be cardholder) payable to: City of Edinburg
  • $35.00 per processing fee
  • $15.00 per set of 2 photos (service optional)
• If paying for more than one applicant, the City of Edinburg fees can be combined in one method of payment.

• If only one parent will be present they must bring a Statement of Consent: Issuance of a U.S. Passport to a Child DS-3053.
