Dance Aberdeen is requesting your permission to post photos and/or videos on the Dance Aberdeen website, social media channels(Including Instagram/facebook, etc) and/or additional publications, such as magazines/print, thank you cards, etc. These photos and/or videos, either in its entirety or portions thereof, may be used for promotional purposes by Dance Aberdeen, in print or in video format for broadcast or non-broadcast purposes. Dance Aberdeen requires your permission for instructors/Dance Aberdeen members/designates to administer/take temperatures using a non contact-thermometer(no physical contact, laser thermometer pointed at wrist), when required, of children upon arrival, during, and/or end of dance classes. This is only used in relation to COVID 19 to help reduce risk and spread and ensure the safety of our dancers*

DANCE ABERDEEN INC. requires disclosure of exposure or illness is in order to safeguard the health and safety of all participants and limit the further outbreak of COVID-19. This Waiver will be kept safely, and personal information will not be disclosed unless as required by law or with your consent.

A Participant (or the Participant’s parent/guardian, if the Participant is under the age of 18) who is unable to agree to the terms outlined in this document is not permitted to participate in the Organization’s activities, programs, or services at this time.
I/We Understand The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization and COVID-19 is extremely contagious. The Organization has put in place preventative measures to reduce the spread of COVID-19 and all participants (or their parent/guardian, when applicable) will adhere to the compliance standards described in the “Dance Aberdeen Inc. Covid Plan and Procedures”.*

I/We understand and attest that the Participant has not, nor has anyone in the Participant’s household, experienced covid-like symptoms within the last 14 days of class(es), if they have, the participant shall not attend class.*

I/We understand If the Participant has been diagnosed/exposed with and/or to COVID-19, the Participant must be cleared as noncontagious by provincial or local public health authorities prior to attending class. If Participant was exposed to a person with a confirmed or suspected case of COVID-19, the date of exposure was more than 14 days prior to the participant attending class.*

I/We understand the Participant has not/will not, nor has any member of the Participant’s household, traveled to or had a lay-over in any country outside Canada. If the Participant travels, or if anyone in or Visiting the Participant’s household travels,outside of Canada, the Participant will not attend any of Dance Aberdeen’s activities,programs or services until at least 14 days have passed since the date of return.*

I/We Understand that the Participant is participating voluntarily and understands and assumes the risks associated with COVID-19. The Participant (or the Participant’s parent/guardian, on behalf of the Participant(when applicable)) agrees to assume those risks, including but not limited to exposure and being infected.*

I/We understand the Participant will follow recommended guidelines within the “Dance Aberdeen Inc. Covid Plan and Procedures”, limiting exposure to COVID-19. This document will remain in effect until Dance Aberdeen Inc, per the direction of the provincial government and provincial health officials, determines that the acknowledgements in this waiver are no longer required. Dance Aberdeen Inc may remove the Participant from participation in the activities, programs or services at any time and for any reason if Dance Aberdeen Inc. believes, in its sole discretion, that the Participant is no longer in compliance with any of the compliance standards described in this document.*
