  • Nomination for Vice-President

    I hereby nominate the following FPM Fellow for the office of Vice-President. (Honorary Fellows are not eligible to be nominated). I note that this office carries trustee and company director status & responsibility. 

  • The Fellow nominated for Vice-President must provide a Candidate Election Statement, available from the election webpage which is submitted with this proposal form.

    Only current FFPM or MFPM are eligible to propose. Honorary, Assoicate and Affiliate members are not eligible to propose or second a nomination.

    Your nomination must be seconded by another FPM Fellow or Member. 

  • Submitting your nomination

    Thank you for nominating for the FPM Vice-President. You will receive a copy of this nomination by email when you submit it.

    You must:

    1 - Send the copy of your nomination to your seconding Fellow/Member, they need to see it before they can confirm they second your nomination. 

    2 - Ask your supporting Fellow/Member to complete the Second nomination form.

  • Data Protection

    The Faculty of Pharmaceutical Medicine (FPM) processes the information provided in connection with your nomination following the principles of the Data Protection Act 2018. The information you provide in this nomination will be shared with FPM's Board of Trustees and in the event that a ballot is required the information provided in Candidate Election Statement will be circulated with ballot papers. Any other sharing of information will only take place with your consent or if FPM has a lawful basis to do so.
