21st Annual Mon Valley HIV/AIDS Awareness Walk Sponsorship Form

SPONSORSHIP LEVELS:

• $1,000 Diamond Sponsor:  Be recognized as a sponsor of the event. Receive an extra-large logo with prominent location on T-shirts, print materials, and web materials. Receive up to 25 T-shirts.
• $750 Platinum Sponsor:  Be recognized as a sponsor of the event. Receive a large logo on T-shirts, print materials, and web materials. Receive up to 18 T-shirts.
• $500 Gold Sponsor:  Be recognized as a sponsor of the event. Receive a medium logo on T-shirts, print materials, and web materials. Receive up to 13 T-shirts.
• $300 Silver Sponsor:  Be recognized as a sponsor of the event. Receive a small logo on T-shirts, print materials, and web materials. Receive up to 8 T-shirts.
• $150 Bronze Sponsor: Receive three-line recognition on T-shirts, print materials, and web materials. Receive up to 4 T-shirts.
• $100 Red Ribbon Sponsor:  Receive one-line recognition on T-shirts, print materials, and web materials. Receive up to 3 T-shirts.

In order to be included on the T-shirt, your sponsorship form and payment must be received by the end of business day on FRIDAY, AUGUST 6, 2021. Please indicate your preferred T-shirt size and quantity below. All T-shirts beyond your contributor level amount must be purchased at an additional cost of $10.00 each. Size availability is not guaranteed for registrations received after August 6, 2021.
