Incident Report Form
This form is for the initial reporting of all incidents and accidents to the club and an incident can be raised any club member. The reported incidents and accidents are sent to club management for review and are recorded in a log. Incidents may be subject to further investigation and reporting as deemed necessary. If you believe your incident is related to a child safeguarding issue then please raise directly with the club Welfare Officer by email to welfare@severnsidetritons.org.uk.
