Reminders:
1. Availability must be maintained for a minimum of 3 months
2. Daytime hours are prioritized for full-time employees who also have PM (12:00-6:00 pm) availability
3. Must include a minimum of 3 afternoons for availability
4. Must include 5 afternoons for full-time schedule request
5. A new Schedule of Availability form must be submitted for any changes at a minimum of 2 weeks in advance of the proposed new availability effective date
6. Consistency is crucial for our client's progress and development
7. The greater the availability you can provide to us, the better the chance the office will be able to create a desirable service schedule
