I am agreeing to let the individual listed above engage in a potentially dangerous activity. I acknowledge that, even if Memphis International F.C. uses reasonable care in providing this activity, the risks involced may result in various types of injury including, but not limited to, the following: sickness, bodily injury, death, emotional injury, personal injury, property damage and financial damage.
In consideration for the opportunity to participate in the above activity, the Participant (or Parent/Guardian) acknowledges and accepts the risks of injury associated with participation in this activity. The Participant (or parent/Guardian) accepts personal financial responsibility for any injury or other loss sustained during the activity or during transportation to and from the activity, as well as for any medical treatment rendered to the Participant that is authorized by the Memphis International F.C. or its agents, employees, volunteers, or any other representatives. Further, the Participant (or Parent/Guardian) releases and promises to indemnify, defend, and hold harmless Memphis Internaitonal F.C. and its agents, employees, volunteers, or any other representatives for any injury related directly or indirectly out of the above activity, whether such injury arises out of the negligence of Memphis International F.C. or otherwise.
Recognizing the possibility of injury associated with soccer and in consideration for Memphis International F.C. accepting the registrant for its soccer programs, I hereby release/discharge Memphis International F.C., their employees, volunteers, and associated personnel, including the owners of fields and facilities utilized for programs, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the programs.
I agree the player listed above has received a physical examination by a physician and has been found physically capable of participating in the programs. In the event of an accident, injury, sickness, etc., under the direction of the Participant listed above, until such time as I can be contacted, I hereby give my consent and permission for any and all medical attention/treatment to be administered to my child. I agree to be responsible for the payment of any such treatment.
I hereby authorize and grant permission to the rights of my child’s/youth’s image, likeness, and sound of their voice as recorded on audio or videotape as well as the use of recorded images or videos of them, with or without their name by Memphis International F.C. for purposes including, but not limited to promotional materials, printed publications, internet posts including social media, television, and other media sources. I understand their image may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product wherein their likeness appears. I understand and acknowledge that I will not receive notice of use of recorded images or videos prior to such use. Memphis International F.C. cannot guarantee any of the above nor can we guarantee the use of recorded images or videos. With full knowledge and consent, I waive all claims for royalties or compensation for use or for damages arising or related to the use of the above my participant's image or recording. I release Memphis International F.C., its officers, trustees, employees, and agents from liability for any claims by me or any third party in connection with the use of their images.
You have the right to refuse to sign this form, and Memphis International F.C. has the right to refuse to let your child participate if you do not sign this form.
